I, of legal age, hereby depose and state that:

1. I agree to allow the Institute for Inner Studies Limited (IIS) to transmit my personal data provided in the IIS system for Master Choa Kok Sui (MCKS) online courses, namely name, address, email address, date of birth, phone number to the Thabo Hollander for student record purposes, namely to save them in the respective Instructor’s or Organizer’s database to create profile about each student;

2. I further give my consent for the release and use of my personal information, namely my name, address, email address, contact number and the MCKS Courses that I will or had studied from my previous Instructors or respective organizers of my attended or future attendance of MCKS Courses to be transmitted to IIS for its record purposes of each student.

3. My personal data will not be transmitted to any other third party without my permission.

4. My consent shall remain in force until I withdraw it. I can withdraw this consent at any time without giving any reasons with effect for the future in writing or by email to info@globalpranichealing.com or to the email address of the Instructor or Organizer mentioned in digit 1.

5. I have read and understood the contents of this Consent for Release and Transmission of Personal Data and Information. I further declare that this consent has been given voluntarily and with full knowledge of its significance.

6. I have read and understood the enclosed Data Protection and Privacy Policy.

 

 

DATA PROTECTION AND PRIVACY POLICY

Using and forwarding Personal Data provided in connection with the Confidentiality and Non-Disclosure Agreement

  1. CONFIDENTIALITY AND NON-DISCLOSURE AGREEMENT FORM

If you complete and sign the Confidentiality and Non-Disclosure Agreement form, the provided Personal Data are collected, stored and processed for fulfillment of the contractual performance. Should the data not be provided, it may make it impossible to conclude the Agreement. The Personal Data entered in the provided in the IIS system for Master Choa Kok Sui (MCKS) online courses (name, address, email address, phone number, date of birth) will only be used to handle and process the Agreement. The legal basis for processing the Personal Data is Article 6(1)(b) GDPR. The Personal Data you provide us with the IIS system for Master Choa Kok Sui (MCKS) online courses will be deleted as soon as they are no longer required for achieving the purpose for which it was collected.

 

  1. CONFIDENTIALITY AND NON-DISCLOSURE AGREEMENT FORM

Your Personal Data will be transmitted to the Thabo Hollander. The Personal Data will not be transmitted to any other third party without your or a legal permission. The legal basis for forwarding the Personal Data is your explicit consent according to Article 6(1)(a) GDPR. You may, at any time, revoke your consent granted insofar with effect for the future. a notification in text form is sufficient (e.g. email to thabohollander@gmail.com or to the indicated email address of the mentioned third party). The lawfulness of the data processing based on the provided consent until the withdrawal is not affected by this.

REVOKE. You have the right to revoke your consent given to us at any time (Art. 7(3) GDPR). If you do so, we will cease processing the data that were based on that consent. Moreover, you have the following rights in terms of your Personal Data:

ACCESS. You have the right to obtain, at any time, information about your recorded Personal Data (Article 15 GDPR).

RECTIFICATION/ERASURE. You also have the right to have your Personal Data rectified or erased (Articles 16 and 17 GDPR).

RESTRICTION OF PROCESSING/DATA PORTABILITY. And you also have the right to restriction of processing of your Personal Data (Article 18 GDPR) and to data portability (Article 20 GDPR).
